S\u2019il \u00e9tait acquis aussi que leurs praticiens \u00e9taient les meilleurs, nombre d\u2019entre eux ont migr\u00e9 vers le secteur priv\u00e9 qui leur garantit des plateaux techniques tout aussi performants. Les classements r\u00e9alis\u00e9s par la presse hebdomadaire, \u00e0 qui cette concurrence n\u2019a pas \u00e9chapp\u00e9, et l\u2019importance que leur accorde le grand public t\u00e9moignent bien de cette situation nouvelle\u2009: \u00eatre bien soign\u00e9 est normal, avec une \u00e9volution d\u2019une obligation de moyens vers une obligation de r\u00e9sultat, disposer des meilleures technologies l\u2019est tout autant.<\/p>\n<p>Alors, comment faire la diff\u00e9rence\u2009? D\u2019abord par un service de qualit\u00e9, c\u2019est-\u00e0-dire l\u2019accueil et la facilit\u00e9 des d\u00e9marches administratives, la disponibilit\u00e9 et l\u2019\u00e9coute des personnels soignants, parfois surcharg\u00e9s de travail, la qualit\u00e9 des prestations \u00ab\u2009h\u00f4teli\u00e8res\u2009\u00bb, pourtant souvent sous-trait\u00e9es et fortement n\u00e9goci\u00e9es.\u00a0 La fluidit\u00e9 du parcours de soins sert autant, voire davantage, la productivit\u00e9 de l\u2019\u00e9tablissement que le patient si ce dernier n\u2019est pas pleinement associ\u00e9 aux d\u00e9cisions le concernant par un personnel m\u00e9dical conciliant efficacit\u00e9 et s\u00e9curit\u00e9.<\/p>\n<p>Un autre \u00e9l\u00e9ment essentiel est la pr\u00e9sence de l\u2019h\u00f4pital hors de ses murs, l\u2019accompagnement des patients avant et apr\u00e8s l\u2019hospitalisation. La relation ville\/h\u00f4pital peut \u00e9voluer gr\u00e2ce au d\u00e9veloppement de nouveaux outils adapt\u00e9s et d\u00e9di\u00e9s. Deux exemples\u2009:<\/p>\n<ul>\n<li>un syst\u00e8me proposant un accompagnement du patient quatre jours avant et quatre jours apr\u00e8s l\u2019hospitalisation\u2009: Easy Hospi\u2009;<\/li>\n<li>la messagerie Lifen facilitant les \u00e9changes entre le m\u00e9decin traitant (g\u00e9n\u00e9raliste et souvent sp\u00e9cialiste), le m\u00e9decin hospitalier, le patient sans oublier les pharmaciens (officine et h\u00f4pital) et les autres professionnels de sant\u00e9.<\/li>\n<\/ul>\n<h1>Pr\u00e9parer le patient \u00e0 son hospitalisation<\/h1>\n<p>Easy Hospi est accessible via un identifiant et un mot de passe fournis par le m\u00e9decin \u00e0 l\u2019origine de l\u2019hospitalisation. Apr\u00e8s la d\u00e9termination de son profil (\u00e9tat de sant\u00e9, intervention pr\u00e9vue, anesth\u00e9sie locale ou g\u00e9n\u00e9rale\u2026), le patient est appel\u00e9 \u00e0 consulter pendant vingt minutes environ chaque jour le site de l\u2019\u00e9tablissement pour aborder les diff\u00e9rents aspects de son hospitalisation qu\u2019ils soient m\u00e9dico-techniques (explication du parcours, de l\u2019anesth\u00e9sie, r\u00e8gles d\u2019hygi\u00e8ne, conseils nutritionnels\u2026) ou pratiques (gestion du stress et de l\u2019anxi\u00e9t\u00e9, pr\u00e9paration de l\u2019admission et de la sortie\u2026).<\/p>\n<p>L\u2019objectif est d\u2019informer le patient et de personnaliser son accompagnement du retour au domicile, y compris dans certains aspects difficiles (gestion de la douleur, du pansement\u2026) pour optimiser le retour \u00e0 une autonomie normale. En sus de l\u2019indispensable pr\u00e9sence des professionnels de sant\u00e9, ce type de programme permet au patient de maintenir un lien avec l\u2019h\u00f4pital.<\/p>\n<h1>S\u00e9curiser et faciliter les \u00e9changes patient\/ville\/h\u00f4pital<\/h1>\n<p>Avec la messagerie Lifen, l\u2019objectif est d\u2019aider le m\u00e9decin \u00e0 prendre en charge le patient plus rapidement et plus efficacement en facilitant l\u2019acc\u00e8s \u00e0 ses comptes-rendus et r\u00e9sultats d\u2019examens. Concr\u00e8tement, cet outil, se pr\u00e9sente sous la forme d\u2019un site Web s\u00e9curis\u00e9 auquel le m\u00e9decin (de ville ou d\u2019h\u00f4pital) se connecte gr\u00e2ce \u00e0 sa carte CPS ou \u00e0 son compte MSSant\u00e9. Pour les m\u00e9decins hospitaliers, un syst\u00e8me d\u2019authentification propre \u00e0 l\u2019\u00e9tablissement est mis en place avec la direction des syst\u00e8mes d\u2019information. Une fois authentifi\u00e9 sur la plate-forme, tout m\u00e9decin peut \u00e9changer avec ses confr\u00e8res.<\/p>\n<p>Pour les h\u00f4pitaux, Lifen est aussi une solution d\u2019externalisation de l\u2019envoi de courriers. Alors que la plupart des comptes-rendus sont aujourd\u2019hui envoy\u00e9s par voie postale, le d\u00e9ploiement d\u2019une telle plate-forme permet, au-del\u00e0 des gains de temps, de ranger, sous contr\u00f4le du m\u00e9decin, les documents \u00e9chang\u00e9s dans un espace de partage s\u00e9curis\u00e9.
